This agreement is between Meadow Lark Agency, Inc. of Billings, MT and your company

Meadow Lark Agency, Inc. agrees to pay the carrier TWO business days from reciept of the freight bill and proof of delivery. Payment is dependent upon receipt of all appropriate paperwork and that there is no research that needs to be done concerning the invoice.

By selecting the "1.5% 2 Business Days" line below, the carrier agrees to have Meadow Lark Agency deduct a 1.5% fee from the carrier's total freight bill. By selecting the "Net 30 Day" option, the carrier declines this early payment offer and elects the normal payment term of 30 days from reciept of carrier's paperwork.

This signed contract will be an addendum to the original contract signed between teh above carrier and Meadow Lark Agency, Inc. Either party may terminate this election at any time with written notice. If Meadow Lark receives notice of assignment from a factoring company for carrier invoices, this contract becomes null and void.
